Pursuant to Executive Order (EO) No. 139 dated August 18, 2021, amending EO 115 dated August 5, 2002, barangay officials who died during their term of office are covered by death benefit claims nationwide.
- Beneficiaries of a deceased Punong Barangay can receive P20,000 for death benefit and Php 2,000 for burial expenses.
- Beneficiaries of a deceased member of a Sangguniang Barangay, including its Sangguniang Kabataan (SK) chairpersons as ex-officio members, barangay secretary, barangay treasurer and Indigenous People Mandatory Representative (IPMR), can receive Php10,000 death benefit and Php 2,000 for burial expenses.
What are the requirements:
- Application for Barangay Official's Death Benefit Claim (4 copies)
- Certified True Copy (CTC) or Original Death Certificate (1 CTC/Original and 3 photocopies)
- Photocopy of Barangay Official's Information Sheet (BOIS) (4 photocopies)
- Certified True Copy (CTC) or Original Oath of Office (1CTC/Original and 3-photocopies)
- Certified True Copy (CTC) or Original Marriage Certificate, if claimant is the widow/widower of the deceased barangay official (1 CTC/Original and 3 -photocopies)
- Certified True Copy (CTC) or Original Birth Certificate, if claimant is a son/daughter of the deceased barangay official (1 CTC/Original and 2 photocopies)
- Certified True Copy (CTC) or Original Birth Certificate of the deceased and Affidavit of Two Disinterested Parties, if the deceased barangay official is single and the claimant is his/her parent/brother/sister (1CTC/Original and 3 photocopies)
- Waiver of Rights, if there are other claimants in the BOIS;
- Photocopy of Government issued Valid IDs of claimant and deceased official (4 photocopies);
- Certification of Incumbency (from MBB)
- Service Records (from MBB); and
- Citizen's Charter Service Request Form
